The Kübler-Ross Model for ACOs – moving from denial to acceptance
ACOs as a concept have existed for years although the term is relatively new. Since 2009 providers have been watching ACOs initially with a high degree of denial and eventually have come to acceptance of the concept. That being said, each ACO is different and it will be very confusing to understand the strategies given the diversity in the marketplace. Also, local market factors and implementation are going to have dramatic impact on actual results further clouding the issue. This talk will explore the evolution of ACOs and how to successfully manage populations. The field is evolving rapidly and there are still many areas of unknowns and unexplained variability.

Smart ACO Management and Governance
Managing the health of populations will require a fundamentally new business model that involves new governance, management teams and operations. It is natural to think of population health as an extension of standard hospital operations, but the reality is that population health management is a different business model. Many of the value based payment programs can provide seed funding and direction to set up the infrastructure that is required to operate in this business model. This session will explore the changes in the market that are happening today to prepare for authentic population management and what are the leading practices in the new models that are likely to result in success.

Population Health Operations
There is commonly a gap between strategy and operations and with population health management this variance is akin to a black hole. This session will explore key tips that ensure operational success (or at least the avoidance of complete disasters) Population Health Operations.

Operationalizing Population Health Management
Many health systems have strategies for population health management that are impossible to operationalize. While at times the strategy seems certain, in many cases there was not a clear understating of the realistic market opportunity and organizational capabilities to deliver on the strategy. This session will explore current strategies health care systems are taking across the country and explore areas of success and failure. We will also discuss local market factors that are a key determinant in the type and quantity of your impact opportunity with population health management.

Population Health Management – From Strategic Fantasy To Operational Reality

The title 'volume to value’ has become a hallmark of just about every health care discussion today. While the strategy is something that we can understand, the challenge is delivering on the promise of moving from volume to value. If we are able to see the current and desired state, why is the transition so challenging? This session will explore the specific local market factors that can impact strategy and lead to re-framing what your own realistic market opportunity. We will also outline key operational elements of organizations that are delivering value based care and how you can learn from them to both improve the care for a population and reduce the total cost. We will also explore the challenges for a volume based health care system to adopt these practices and discuss a realistic path forward.
